#1 - SUPPORT WORKER

REPORT TO – SERVICE MANAGER/ASSISTANT MANAGER

Job purpose – To plan, monitor and deliver the highest standards of personalised support to services users, ensuring that everyone is treated with respect and dignity, and that individual’s rights to privacy, dignity, independence and choice are met.

Key Responsibilities

1. To provide a high level of support and attention to service users, following individual care plans carefully and ensuring that all contact is polite, friendly, warm and supportive. To act as a key worker for named services users, ensuring that these responsibilities are carried out in full (as defined in the training manual) and to the ethos of PCS.

2. Responsible for; understanding and complying with statutory and legal requirements that are relevant, including; Health & Safety ,COSHH and all aspects of the Health & Social Care Act to ensure that a safe environment is maintained throughout the homes.

3. To participate in the formulation of individual support plans and weekly plans for service users as well as monthly or weekly reports.

4. To always be alert to service users’ physical and mental wellbeing, immediately dealing with and reporting any changes in a services user’s condition. Liaising with other care professionals as required, and ensuring that all records are updated accurately.

5. To respect and maintain confidentiality of service user’s personal information at all times, this includes service user’s behaviour and actions and any incidents that may occur in the course of day to day care/support and to ensure that all support staff understand and respect this.

6. To be patient towards service users and their needs, and encourage, assist and support service users to participate in leisure activities as appropriate to the individual, ensuring their social and emotional needs are met. Escorting service users to outside appointments and leisure outings.

7. Undertaking general cleaning duties throughout the homes as needed.

8. To ensure thorough daily logs and handover records are completed on each shift.

9. To have a flexible working approach on a 24/7 basis this will include weekends duties and sleep-in shifts.

Job context
This role is a team role, with clear responsibility for specified service users. Clear communication with team members is essential, as well as reporting to the Service Manager or Assistant Manager.

To ensure the home values of trust, respect and care are always maintained and to treat service users in the way that they would expect to be treated themselves.

Key measures
Mandatory training is up to date
Supervision meeting with Service Manager every 2 months

Person Specification
Some experience in care or working with people
Good numeric and literacy skills/I.T skills
Basic budgeting skills
A warm and friendly disposition
Strong team player
Good communication skills, able to put others at ease
Reliable and punctual
