As an established provider we adopt the person-centered approach for service users, their families and friends and for our staff. We work towards building an open and productive relationship with commissioners, local authority care managers, social workers and the community.
OUTREACH SUPPORT
OUR ACCOMMODATION
We aim to provide supported living accommodation including support, for adults over the age of 18 with mild-moderate mental health, learning difficulties and associated needs who may need a “platform” to reach their personal goal of independent living.
Our support is focused on delivering the best outcomes for our service users with person-centered support. Service users will be treated with respect and dignity and our support is individually tailored to retain and maximise their choice and control and to reflect what is important to them.
To deliver this personalised service we “listen”.
We all have dreams, ambitions and goals and at Platform we believe that the individuals who choose to use our services are entitled to the same rights, responsibilities and opportunities as anyone else; are entitled to be listened to, guided and supported in achieving their dreams and goals, thus enabling them to live independent lives in the community. We emphasise the importance of ensuring that all who use our services are treated equally.
The development of Platform Care Services Ltd is based on our belief that if appropriate packages of support are available to people who would otherwise be unable to live within a family environment then they will be able to do so with us.
The support workers are part of the team that supports and sustains placements by identifying and meeting the needs of individuals. Through partnership with others, support workers provide focused, practical input to help sustain placement stability and deliver good outcomes in the key areas of safety, health, enjoyment and achievement, making a positive contribution and achieving economic well – being.

We work with service users to identify their individual needs and work towards these needs being met. We are committed to ensuring that our service users are able to access opportunities in the local community, utilising local support networks and local authorities.
Placement types include;
- Long Term Placements – a placement that is planned (prior to placement commencement or subsequently) to continue to be available for at least two years.
- Short Term Placements – a planned placement that is made for any purpose, which is not intended to provide a long-term placement but to provide support and accommodation until an individual is ready to move on.
- Short Term Breaks – (subject to availability) are available for those people who require regular short breaks. These will be planned and be with an identified, specifically matched and appropriately trained support worker.
Platform Care Services also offers support within the community or within an individual’s home should they not require accommodation.
Support can be provided for any ages or needs following a thorough assessment of the needs and support required.
Types of support may include;
- Support with life skills or around the home with cleaning and cooking
- Support to find employment or education
- Support out in the community; attending appointments or activities Support to complete day to day tasks like shopping
We currently operate supported living homes across Woodley, Earley, Purley and Purley-on-Thames, providing safe, comfortable and well-located accommodation for individuals who require support while maintaining independence.
Our properties are carefully selected to ensure excellent access to public transport, local shops, healthcare services and community amenities, enabling the people we support to remain active and connected within their local community.
Accommodation
Each individual living in our homes is provided with their own private bedroom, offering privacy, comfort and a personal space to call their own. Residents are given their own set of keys to their bedroom and the front door, supporting independence and security.
Residents also share welcoming communal areas within the home, including:
- A comfortable lounge
- A fully equipped kitchen
- A dining area
- Shared bathroom facilities
Outdoor Space
All of our properties include private garden areas, providing a relaxing outdoor environment where residents can enjoy fresh air and social activities. During the warmer months we organise outdoor activities such as summer barbecues and garden gatherings, encouraging social interaction and wellbeing.
Staff Support
Each home includes a dedicated staff room with facilities for overnight staff. Our houses operate with sleep-in staff who remain on site overnight, ensuring that support is always available when needed. Residents can contact the on-site sleep-in staff during the night in case of emergencies or if support is required.
